Closing the Gap After the Consultation
Nuevo Care is the clinical operating model behind Nuevo Medical. It exists to address a specific problem: in a conventional arrangement, a patient receives sound medical advice and is then left to independently coordinate the investigations, allied health, medication changes, lifestyle support and follow-up that advice depends on. Some of that happens. Some of it doesn't, and the result is delayed care, poor adherence or incomplete follow-through.
Nuevo Care is designed to reduce that gap. It connects general practice, cardiology, fertility and reproductive health, allied health, radiology, pathology, pharmacy and structured health programs around clear care pathways within one building.
The purpose isn't to replace a practitioner's clinical judgement. It's to provide the systems and services that help patients move from advice to action.
02The pathway
How the Model Works in Practice
The process is intended to be simple, and it runs in six stages.
GP assessment
Your GP identifies clinical needs, risk factors, goals and the practical barriers to following through. This is the point everything else runs from.
Care pathway identified
Your GP determines whether additional support would help, such as allied health, diagnostics, pathology, structured programs or specialist input.
Referral and onboarding
Where appropriate, you're referred into the relevant pathway. Our team supports the booking, onboarding and communication so care isn't lost after the consultation.
Allied health and program support
Practical support follows through exercise physiology, physiotherapy, dietetics, clinical psychology, speech pathology or a structured program.
Progress and communication
Updates and feedback go back to your GP and any specialist involved, giving them visibility over what's actually happening.
Review and long-term follow-through
Your GP reviews and adjusts the plan over time, and remains central to your ongoing care.
03Independence
What the Model Does Not Change
Two things are worth stating plainly, because a centre with services under one roof invites an assumption we want to correct. First, practitioners at Nuevo Medical retain full clinical independence. Our GPs, cardiologists and allied health practitioners remain responsible for their own clinical decisions, treatment recommendations and referrals. Second, no practitioner is required to refer internally. Internal services exist to provide convenience and support follow-through, not to capture referrals. Where a patient's needs are better met elsewhere, referral goes elsewhere, and we maintain relationships with external specialists across psychiatry, paediatrics, endocrinology, gastroenterology, orthopaedics, rehabilitation medicine, obstetrics and gynaecology and other fields. Patients retain choice throughout, and all referrals remain subject to practitioner judgement and patient preference.
