For Risk Factors, Not Diagnoses
This program is for people who don't have a diagnosed condition but do have risk factors that have been identified and documented. Our chronic disease prevention program on the Gold Coast runs at Nuevo Medical for exactly that group, combining supervised exercise, dietetic input and scheduled measurement over a defined period in our Bundall facility. Where a condition has already been diagnosed, chronic disease management through your GP is the appropriate pathway instead, and the distinction matters because the two involve different clinical arrangements. What the Program Covers
The program addresses the modifiable factors identified in your assessment.

Baseline measurement
Blood pressure, weight, waist circumference and relevant pathology recorded at entry. These form the comparison set for everything that follows.

Supervised activity
Exercise physiology sessions prescribed from tested capacity. Programs begin at current capability rather than at a general starting point.

Nutrition assessment
Dietetic review where diet is among the identified factors. This is individually assessed rather than delivered as standard advice.

Scheduled review
Measurements repeated at set intervals and reported to your GP. The program is adjusted from those results rather than run to a fixed plan.
03Eligibility
Distinguishing Risk Factors From Diagnosed Conditions
The starting point for this program is a GP consultation that establishes what has actually been identified. Your GP reviews your history, family history, current measurements and pathology, and determines which risk factors are present and whether any have crossed into a diagnosis. That distinction determines the pathway, since a diagnosed condition involves clinical management rather than a prevention program. Bringing previous blood results, particularly older ones, helps your GP assess the trajectory rather than a single point. Where the program is appropriate, your GP sets out what it should focus on, and the allied health assessment follows. Progress is reported back at scheduled intervals, and your GP reviews whether the arrangement remains the right one.

FAQs
Prevention addresses identified risk factors in someone without a diagnosed condition, while management applies once a condition has been diagnosed. The distinction determines which clinical pathway is appropriate.
Modifiable factors are those that can potentially change, including blood pressure, lipids, weight, physical activity and smoking, as distinct from factors such as age and family history. A GP assesses which are present individually.
The period is set at assessment based on what's being addressed and reviewed at defined intervals rather than fixed in advance. Review points are scheduled at the outset.
